Thankfully, when I rode through the NADA Tunnel, the opposing traffic could see my headlight from the other side.   They weren't at the entrance when I entered.   Had they been there, one of us would have had to wait.   My understanding is, who ever is willing to wait signals by flashing their lights.   If, you were to meet someone in the middle, then both parties obviously weren't paying attention to oncoming headlights.   It's totally dark in that tunnel so entry requires lights.  Obviously, it's not a perfect system but, it seems to work.

Been through it many times .You want a RUSH go through Dingus tunnel in West Virginia. It's the one Hubcap posted.

Long dark Tunnels with curves can be very disorienting.
