In the early ‘90s, when I was stationed at Caserma Carlo Ederle in Italy, it was very common to see soldiers riding bicycles back and forth to work.  So it came as no big surprise that, after a series of painfully comic accidents, a new policy was announced, saying in summary, “Soldiers shall no longer salute officers who are engaged in the riding of a bicycle.                    Michael Teas

True story.

I applied for and was given a direct appointment to USAFR as a Judge Advocate.  We did not do a basic course, just a two week custom and courtesy school.   Only days after beginning active duty, I was walking to work from my BOQ when I saw a bird col coming right at me on his bicycle, in a flight suit with hat on.  I rapidly scanned my memory of protocol but came up blank; except for the rule that said when in doubt, salute.  So  I did, and he stopped and got off the bike and read me the riot act.  "Don't you know not to salute someone on a bicycle... someone can get hurt trying to return the salute"  "Yes, sir.  Sorry, sir."   Some folks apparently need two hands to ride a bicycle.
