After installing my Kurykan dial clutch and brake levers my brake lever hasn't been fully returning to the extended position to turn off the brake switch, so before I rebuild the master cylinder, i thought I'd bleed again. (I did try to loosen the acorn nut and screw that holds the lever on with no luck).

I'm using a mityvac to bleed my brakes. The left side has perpetual air coming out. Is this usually from air coming in from the bleed screw? Where else could it be?

Have you guys seen the brake levers having insufficient return force and require a rebuild of the master cylinder?

Can't help with the lever question, but that's common about the air bubbles.  It comes in around the threads on the bleed screw.  You can wrap the bleed screw in teflon tape, but be careful not to cover the holes; just the threads.

In Mityvac's instructions the address the air bubble problem.  The vacuum will draw air in around the bleed screw threads.  Removing, lubricating and reinstalling the lever pivot will probably solve your problem.
