Has anyone had this happened ?
1999 standard, the clutch works perfect , no slipping. And it goes in every gear with no problem, except when I come to a stop i leave the bike in gear,  after a minute or so the bike starts taking off while Im still holding the clutch level in.

Sounds like the Clutch Master Cylinder needs rebuilt.
You are "slowly" losing pressure in the line while sitting there, and it is allowing the clutch to re-engage.

Could it be as simple as just needing a flush? Fresh oil and removal of any contaminants that might be causing problems of the master cylinder plunger.

My first suggestion also would be to change the fluid and give it a good bleeding.

 I changed the slave cylinder,  thinking that maybe that was the problem  and flushed it.

 The slave cylinder was leaking but I ready rebuilded  but that didn't fix the problem .

If the clutch lever works smoothly, freely and has full travel, and the clutch assy engages smoothly then I doubt there is anything wrong with the clutch itself. If it is starting to get 'jerky' upon release of the lever then there may well be a few sheared rivets. If that isn't happening then I would think there is just a hydraulic issue somewhere. Flushing and changing fluid means different things to different folks. Some folks have had a hard time getting all the air out of the system. There is also the possibility of a bad cylinder. I just think  a good flushing at first is a good place to start.
