Oh my! So many possibilities to both of those questions! I'm afraid one could sit there for a week just trying to figure out which would be the best answers! I can hear myself now, asking "Hey, I messed up the other 4 forms, can I have just one more? Can I write on the back? Does anyone really read these things?". Hmmmm. Maybe, one could circle the word "race" and put "see answer under "ethnicity"". Then, under "Ethnicity", put "see answer under "race". Or how bout  "Race" "why, yes!, yes I do!" and under "ethnicity" put "No thank you".

I enjoyed that Daniel.  The ethnicity question in the medical sense is there because some cultural backgrounds are more prone to certain diseases than others.  However, I'm such a mixture that I usually fill in MUTT.   :)

FWIW, I don't like these questionnaires either, and esp don't like the idea of them becoming Federal records instead of MY (Privacy Act) medical records, which a govt agency can get only after showing probable cause sufficient for a warrant by a judge. 

But it turns out my ex (of perfect lifetime health), has a lifetime, marginally-high bilirubin count (which might show liver or other problems).  But as soon as the doc saw she was Turkish..... oh, high biliruben (benign) is widespread in Middle Easterners.   So some question about origins may be medically a good idea.
