Something like this. I'm being lazy, looking for something that my staff can do so I don't have to.

I used Excel to make a few for a client some years ago. Once you make one, save as a template and substitute each procedure as needed. I don't think I saved the template but I will look at the back-up tapes to be sure.

Sorry, I probably deleted it along with other like items that I don't use regularly.
